To name KCCK a beneficiary on your employee benefit plan, request a beneficiary form from the administrator of your plan. If KCCK is the sole beneficiary then simply list as follows: The Kirkwood Community College Foundation for the benefit of KCCK 6301 Kirkwood Blvd. SW Cedar Rapids, IA 52404 Tax ID # 23-7076632. If KCCK is to receive only a percentage or set amount from the plan, you would designate that accordingly.
